You will be assigned a dedicated care manager who will act as your single point of contact within the service. Your care manager will ensure that any changes or requests are incorporated into your personal wellness plan and communicated with the rest of your team.
Your dedicated care manager will act as a liaison between external care providers such as hospital consultants, existing homecare providers, HSE/public healthcare providers, on request and with your permission, to ensure all professionals involved in your health and wellness are aligned and working towards the same goals.
A holistic assessment will be carried out on the commencement of your membership to create a personalised wellness plan and this will be reviewed continuously. It will be conducted both in person and virtually. We use a globally recognised assessment process to make the perfect wellness plan for you.
Our in-house GP specialises in medicine for the older person. He leads a highly skilled team of clinicians including doctors, nurses, physiotherapists and occupational therapists. All of these experts will be there for you as required.
Our healthcare experts can contact you virtually but they will also be available in person should it be required.
We believe that prevention is better than cure and our team will actively monitor you for key risk areas and put in place any preventative programs that they believe are necessary.

We also have trusted partners who provide dental services, counselling and psychotherapy, dietetics, mobile diagnostic scanning and acupuncture at negotiated discounted rates. These can be coordinated for you as part of your memberships for an additional fee. You can enhance this service further from our bolt on service menu.
We will work with you to identify the best use of your VIC Nurse led Personal Assistance.
You will have access to 7 hours per week of in-home support from our hand-picked, fully vetted and trained home assistants. Our assistants are supervised by our community nurse who continually monitors the care provided.
You will direct how the hours are distributed and the type of services needed. We do need notice of any changes on an ongoing basis so that we have time to train, risk assess and prepare assistants accordingly.
We will do our best to maintain consistency in the assistant provided and endeavour to foster a long term, trusting and rewarding relationship between our clients and their chosen assistant.

You will receive an hour per week of assistance on the go. We will provide you with the necessary support you may need outside your home while social distancing is a challenge. Examples of where this might be useful for you are grocery shopping, personal shopping for gifts, post office visits or dog walking.
It is more important than ever to ensure that we are keeping our bodies healthy and maintaining a nutritious diet. These will be selected from a menu in advance so you can choose your favourites. Our catering partner will provide guaranteed home cooked meals with no synthetic additives or flavouring, zero refined anything- just a combination of quality proteins, vegetable and carbohydrates; all naturally designed to nourish you from the inside out. Easy to swallow options are available if that’s needed.
You will receive 2 hours of dedicated recreational time per week with a member of our team. There will be a varied social, physical and health program available for all members. It will be developed and amended to suit the diverse needs and interests of our members. We will tailour an individualised recreation program for each member and will provide all the equipment needed.
You will have access to our Carer Hub. With the members permission we provide a family/carer portal which we update continuously regarding the members wellness plan and general day to day care requirements. We host monthly Q&A seminars on a variety of topics such as managing and understanding dementia, maximising statutory benefits, coping with stress and tax benefits for care.
As Covid restrictions ease there will be more opportunities to socialise at a distance. You will obviously want to continue to protect yourself during this transitionary period. Protective equipment will be available as appropriate for use by our VIC Social members at our Wellness Centre.
